old mt needs help - 1shopper

How do you type non ST elevation MI.  When I started decades ago it was non-ST-elevation, but got QA's and they took out the second hyphen.  When I looked it up, it is both ways.  Any input would be appreciated. 

Oh and another good one from QA on a nonverbatim account:  The patient has a PAST HISTORY of MI in the PAST.  Took out in the past and she put it back in. Here is another:  ----- mL of Xylocaine WAS injection and she put it back to WERE... so you see why I am questioning. tx
