and they are all in line with the OP's understanding and the BOS 3 citation, although 2 of them say that the comma is optional, so could go either way.
As far as MTSOs and BOS 3, I worked for a MTSO from December 2011 until May 2015 and they used BOS 3. My present MTSO since May 2015 also uses BOS 3. I'm pretty sure more MTSOs than you think use BOS 3 either in whole or in part with some of their own modifications.
