The worst tracking episodes were when I had extremely long reports to transcribe for an ESL who could not speak English and required searching old reports to try and decipher what was oozing out of his mouth. So your BS about "taking an hour to look up doctors" is just that - BS. Why are you so challenged at sticking to the facts?
