is fill in the blanks, which no one would be able to decipher, with what normally goes there, for example, they dictate "bowel sounds no" and QA fills in "bowel sounds normal" where an ESL may be saying "Bowel sounds: No" or that there are no bowel sounds. I have seen this done many times by QA to not leave blanks, yet it can be a critical error.
