When they call the patient by the wrong name or gender or dictate an exam of the right side and then the right side again, or give the date as April 5, when it's actually August 5, etc.

A while back, I had the incorrect spelling of a patient's name in the note. Not my fault, because I had no patient list, and the name was a usual name that was spelled oddly, like I've never seen before. After that, I received a detailed email about how patient's names need to be spelled correctly, and I have to be careful, blah, blah, blah. Meanwhile, doctors usually can't tell the difference between Marie and Maria, Diana and Diane or even Deanna.
