I think it should be that way too, and that's how it has been, and I have always changed a.c. and q.h.s. to before meals and at bedtime (and I still do this).
BUT none of these three is any longer on the NTS dangerous abbreviation list that I can tell, not q.h.s., not h.s., not a.c., so I am still doing it but I wouldn't put it past them to suddenly decide to take off points for this and I refuse to worry about it frankly.
