Maybe you should tell the doctor to put some care into his dictation! Like don't speak at 100 miles per hour and speak clearly. Heck, I have heard some of this mush mouths speaking clearly to their friends prior to, during or after their dictation! Some of these doctors have full blown lists!
And I am not going to transcribe "what he usually says" as that opens me up to QA deciding they heard what they doctor actually said and it was different. That is a side effect of their 99.6, 2 minor errors, you flunk policy. The blanks will end when I don't have to second guess what QA might mark me off for.
