I thought QA had to listen to ENTIRE report? - I had a critical error for

leaving a drug off the meds list.  I went back in and decided to relisten myself to entire report as there were some other issues as well.  Turns out the dictator at the end of the report said "go back and remove HCT from the meds as the pt isn't on that anymore."  

And so I obviously removed it from the list.  QA marked it as a critical omission.  Of course, I asked for reversal and I'm sure I'll probably get it, but the point is that I thought QA were supposed to listen to the entire report for just such instances.  If I had NOT relistened to the whole thing myself, I would have thought that I did indeed omit that drug and would have just accepted the critical error.
