I'll tell you how I did it, I did about 500 an hour when there was work, at transcend I did about 650 before nuance bought them. I use dragon to echo dictate, I listened to speech at 200 percent speed, I use my macros (posted one below you can grab) and others you can find at wordmacros dot net, used autohotkey to call the macros and used the touch pad on my laptop as my mouse so I can keep my fingers constantly at the ready position to type. The dragon medical hurt my pocketbook but it paid for itself over and over since I've owned it, I have DM 10. I hooked a 21 inch monitor to my laptop and set at eye level, the laptop is fast, a must for dragon.
