In other industries, remote employees do not expect to be able to walk the dog during duty hours, unless they do it on a regular break.

With my employer, which is not an MTSO, no matter what job you have, it is exactly the same as working on site. You do not get to remote unless your work quality and productivity meet certain levels, and if they drop you are brought right back into the office. When you are working, you are expected to be working and be available to perform your duties. If you are away from the computer more than a short while, the network logs you out for security reasons.

The attitude of many MTs that they can work at home to enjoy all the benefits of life at home like watching TV and listening to the radio while working, caring for children, home-schooling, stopping to drive the kids somewhere, cooking, housecleaning, laundry, yakking on the phone, and everything else, is one of the factors that dragged this down from a respected profession to what it is today.

Your job needs to be a job, not something you fit in when your home life isn't making demands on you.

Just a suggestion, but if your IBS is so disruptive, you might want to consider a gluten-free diet. It can really help, to the point of eliminating it. Google it.
