Go to google.com enter your state name + small claims court.
I live in California and that is what I did. They give the steps to take, forms to print out. Here, it cost $30 filing fee, filled out the forms, mailed in to the court house, got a court date, had the papers served. No attorneys needed. Any questions look in the phone book for your court house, mine is superior court, call clerk and ask about how to handle if company in another state. Don't sit on it any longer. There are time limits to file, a year, I think but check it out.
I would contact the attorney general in the company state also, and send that same letter to the IRS. Keep all emails as proof and invoices you DO NEED to show proof. State to them, what actions you need to take since no payment received. Outline (email) or mail signed receipt thing, and state the steps considering. This just may stop the whole thing and get paid.
Even consider contacting the hospital you did the work for. This might get them to find a way to pay you. This is happening far to many MTs. Read this a lot on this board. Thank goodness this board is available, since we are out of sight, they think HEY what can they do.
So sorry this has happened.
